For Research, Not So Much."},"content":{"rendered":"\n<p>Okay, okay, I know \u2013 science is supposed to be unbiased. And a responsible scientist will do her best to check her opinions at the door. But honestly, I love a good research article that confirms something I already know. Which is why I enjoyed \u201c<a href=\"https:\/\/ejtcm.gumed.edu.pl\/articles\/64\" target=\"_blank\" rel=\"noreferrer noopener\">Sciatica: Internet Search Trends<\/a>\u201d more than I probably should.<\/p>\n\n\n\n<p>As research papers go, this one was fairly easy to follow. The authors checked sites commonly used by patients to research health conditions (Wikipedia and Google) against the published article record in <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/\" target=\"_blank\" rel=\"noreferrer noopener\">PubMed<\/a>.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">The Public View<\/h2>\n\n\n\n<p>Because the authors searched publicly accessible platforms, I was able to recreate their data. First, I turned to <a href=\"https:\/\/trends.google.com\/trends\" target=\"_blank\" rel=\"noreferrer noopener\">Google Trends<\/a> to examine search trends.<\/p>\n\n\n\n<p>Unsurprisingly (to me, anyway), the authors reported that searches for \u201cSciatica\u201d and the related term \u201cLow back pain\u201d have been increasing over time. My thought was, <em>of course!<\/em> <em>But doesn\u2019t that just prove we rely on Google more for <\/em>everything<em> now?<\/em><\/p>\n\n\n\n<p>Not necessarily. The authors noted that searches for other neurological conditions have actually decreased. They used \u201cMultiple sclerosis\u201d as a comparison.<\/p>\n\n\n\n<p>When I checked, I saw that sure enough, sciatica and LBP trended up, while searches for MS decreased.<\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"1024\" height=\"573\" src=\"https:\/\/myuprightlife.com\/wp-content\/uploads\/2020\/05\/Sciatica-web-search-history-1024x573.png\" alt=\"\" class=\"wp-image-319\" srcset=\"https:\/\/myuprightlife.com\/wp-content\/uploads\/2020\/05\/Sciatica-web-search-history-1024x573.png 1024w, https:\/\/myuprightlife.com\/wp-content\/uploads\/2020\/05\/Sciatica-web-search-history-300x168.png 300w, https:\/\/myuprightlife.com\/wp-content\/uploads\/2020\/05\/Sciatica-web-search-history-150x84.png 150w, https:\/\/myuprightlife.com\/wp-content\/uploads\/2020\/05\/Sciatica-web-search-history-768x429.png 768w, https:\/\/myuprightlife.com\/wp-content\/uploads\/2020\/05\/Sciatica-web-search-history-1040x581.png 1040w, https:\/\/myuprightlife.com\/wp-content\/uploads\/2020\/05\/Sciatica-web-search-history-640x358.png 640w, https:\/\/myuprightlife.com\/wp-content\/uploads\/2020\/05\/Sciatica-web-search-history.png 1497w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><\/figure>\n\n\n\n<p>It\u2019s worth noting that Google Trends doesn\u2019t show the exact number of searches, just the comparative popularity of a search term over time. So we know that there are more searches for \u201cLow back pain\u201d than \u201cSciatica\u201d, but we don\u2019t know how wide the gulf is.<\/p>\n\n\n\n<p>I also checked a few other common neurological disorders, to see how they would compare. I concluded that the term \u201cMultiple sclerosis\u201d did seem cherry-picked to provide a nice counterexample to \u201cSciatica\u201d and \u201cLow back pain\u201d, though I wouldn\u2019t call it deceptive. The other results don\u2019t prove the point as well, but they don\u2019t paint a dramatically different picture either.<\/p>\n\n\n\n<p>\u201cAlzheimer\u2019s disease\u201d, \u201cEpilepsy\u201d, and \u201cCerebral palsy\u201d held reasonably steady over time, though each had peaks and valleys. \u201cMigraine\u201d rose in tandem with sciatica and LBP. The most interesting pattern was for \u201cAutism\u201d, which spikes reliably every April.<\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"1024\" height=\"531\" src=\"https:\/\/myuprightlife.com\/wp-content\/uploads\/2020\/05\/image-1024x531.png\" alt=\"\" class=\"wp-image-320\" srcset=\"https:\/\/myuprightlife.com\/wp-content\/uploads\/2020\/05\/image-1024x531.png 1024w, https:\/\/myuprightlife.com\/wp-content\/uploads\/2020\/05\/image-300x155.png 300w, https:\/\/myuprightlife.com\/wp-content\/uploads\/2020\/05\/image-150x78.png 150w, https:\/\/myuprightlife.com\/wp-content\/uploads\/2020\/05\/image-768x398.png 768w, https:\/\/myuprightlife.com\/wp-content\/uploads\/2020\/05\/image-1040x539.png 1040w, https:\/\/myuprightlife.com\/wp-content\/uploads\/2020\/05\/image-640x332.png 640w, https:\/\/myuprightlife.com\/wp-content\/uploads\/2020\/05\/image.png 1393w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><figcaption>I can only conclude that autism spikes in April.<\/figcaption><\/figure>\n\n\n\n<p>I had to spend some time getting up to speed on the Wikipedia <a href=\"https:\/\/tools.wmflabs.org\/massviews\/\" target=\"_blank\" rel=\"noreferrer noopener\">Massviews Analysis<\/a> tool, but once I did, it was actually pretty cool. The data only goes back to July 2015, which appears to be why the authors of the paper used a different time frame for the Wikipedia search.<\/p>\n\n\n\n<p>Again, I was able to confirm their results. The Wikipedia page for Sciatica is indexed under four categories, and the rankings in each were as follows:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li><a href=\"https:\/\/en.wikipedia.org\/wiki\/Category:Orthopedic_problems\" target=\"_blank\" rel=\"noreferrer noopener\">Orthopedic problems<\/a>: 1 of 29 pages<\/li><li><a href=\"https:\/\/en.wikipedia.org\/wiki\/Category:Peripheral_nervous_system_disorders\" target=\"_blank\" rel=\"noreferrer noopener\">Peripheral nervous system disorders<\/a>: 2 of 59 pages<\/li><li><a href=\"https:\/\/en.wikipedia.org\/wiki\/Category:Symptoms_and_signs:_musculoskeletal_system\" target=\"_blank\" rel=\"noreferrer noopener\">Symptoms and signs: musculoskeletal system<\/a>: 1 of 25 pages<\/li><li><a href=\"https:\/\/en.wikipedia.org\/wiki\/Category:Pain\" target=\"_blank\" rel=\"noreferrer noopener\">Pain<\/a>: 1 of 135 pages<\/li><\/ul>\n\n\n\n<p>I was less surprised by the rankings, and more surprised by how sciatica pulled so far ahead of the pack. In the \u201cPain\u201d category, \u201cSciatica\u201d averaged 3,141 pageviews a day, compared to 1,884 daily pageviews for the second-place page \u201cAnalgesic\u201d.<\/p>\n\n\n\n<p>This was all a very interesting trip down a data rabbit hole, but why should we care? Basically, this data shows that sciatica and low back pain are both popular topics among the public, and indicates that interest is growing.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">The Research<\/h2>\n\n\n\n<p>I turned my attention to PubMed. Here, it was interesting to see the comparative results for \u201csciatica\u201d versus \u201clow back pain\u201d. (I also threw in \u201cleg pain AND lumbar\u201d so that I could properly compare my results to the authors\u2019.) Three guesses as to which was most popular.<\/p>\n\n\n\n<p>While the authors only looked at the number of articles published in 2018, I expanded my search so I could examine the trends over time.<\/p>\n\n\n\n<p>Most surprising to me was the discrepancy between the number articles on \u201clow back pain\u201d versus \u201csciatica\u201d. If I had any doubts about which topic was the unloved child, they were laid to rest. From 2010-2020, there were twelve times as many articles on \u201clow back pain\u201d.<\/p>\n\n\n\n<p>On a side note, I get annoyed when medical professionals insist I have \u201clow back pain\u201d not \u201csciatica\u201d. I get that \u201clow back pain\u201d is a broader term, but I can\u2019t resist thinking that its prevalence has served to erase an important distinction.<\/p>\n\n\n\n<p>Next, I wanted to see how LBP &amp; friends compared to other neurological conditions in terms of research output. This chart gets a little messy, so bear with me. Alzheimer\u2019s was king among this cohort. (Since most research is funded by government entities, this probably, \u201cThese old people are too expensive.\u201d)<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">What Does This Mean?<\/h2>\n\n\n\n<p>\u201cSciatica\u201d is a subject of great interest to the public, but for researchers, it\u2019s barely an afterthought. Scientific interest in sciatica is dwarfed by that in the broad but nebulous \u201clow back pain.\u201d And interest in low back pain is in turn dwarfed by that in other neurological conditions. (There\u2019s always a bigger fish.)<\/p>\n\n\n\n<p>While I didn\u2019t dive into this claim, the authors note that YouTube videos for searches related to sciatica often yield low-quality information. From personal experience, I know that many top results on sciatica contain outdated, misleading, or unhelpful information. I also know that, if you are Googling sciatica at all, it is probably with some degree of desperation, and you deserve better than a paid ad.<\/p>\n\n\n\n<p>I certainly hope that in the near future, the research will increase to match the public need. I also hope that scientists and science writers do more to promote updated and comprehensive information about sciatica and related treatment options. And while I\u2019m at it, I hope that someday our cars will run on children\u2019s smiles and spam callers will only dial each other.
